Requirements

• All subjects must be clean, sober, and of adequate physical and mental health for any procedure received.
• A state or federally issued form of picture identification must be presented for every procedure. 
• All minors must be accompanied by a parent or legal guardian at all times and both minor and guardian must have their own identification for any procedure received. 
• Payment is expected at the time of service with cash, Discover, Mastercard, or Visa being the accepted methods of payment. 
• All payments and procedures are final and not subject to refund, up to and including payments received for drawing time and any deposits cancelled without 48 hours prior notice. 
• Artfully Insane reserves the right to refuse service to any patron for any reason.
